Stents deployed across the gastroesophageal junction for palliating malignant dysphasia can predispose to reflux including to risks of aspiration. A newly developed anti-reflux valve (Dua) was attached to the lower end of an expandable metal esophageal stent and this modification was shown to be effective in reducing GE reflux without interfering with the primary function of the stent, i.e. relieving dysphasia (Dua et al, Gastrointestinal Endows 53:6;603-613, 2001). The ability of the windsock valve to invert at high-pressure gradients between the stomach and the esophagus allows for belching and vomiting. Once inverted, the valve can easily be reverted to its anti-reflux position by swallowing water.
In a prospective randomized study from Europe (Laash HU, et al. Effectiveness of open versus ant reflux stents for palliation of distal esophageal carcinoma and prevention of symptomatic gastro esophageal reflux. Radiology 2002;225(2):359-365), this anti-reflux stent was compared to an “open” stent in 50 patients with lower esophageal cancers. Twenty-four of the 25 patients randomized to the open stent developed reflux symptoms and one patient died within 24 hrs with aspiration after stent placement. Only 3 of the 25 patients with the Dua anti-reflux stent complained of reflux symptoms and there were no deaths.

8/2006 Anti-reflux Plastic Biliary Stent
Duodenobiliary reflux has been considered as one of the contributing factors for biliary stent occlusion. A newly developed anti-reflux biliary stent (Marathon Stent also referred to as Dua Biliary Stent) was shown to remain patent for a longer time as compared to an open stent in a prospective randomized study done on patients with malignant bile duct strictures (Dua et al, Impact of reducing duodenobiliary reflux on biliary stent patency: an in vitro evaluation and a prospective randomized clinical trial that used a biliary stent with an antireflux valve. Gastrointest Endosc 65:819-28, 2007.).
This stent got FDA 510-K clearance for use in the US and is also cleared for use in Canada and Europe.

Hydroflush biliary stone extraction balloon.
This catheter uses mechanical and hydrostatic forces to extract common bile duct stones. It has received a modification of existing device 510K FDA clearance.
