A strategic plan was created and presented to the Health Care Value Council, in addition to creation of a standing Post-Acute Care Committee.

To present Led a process-management group in the development of a 'floating hospice' for FMLH in partnership with Horizon Home Care and Hospice to provide end-of- life care for patients and fam, and designed to be a bridge the planned inpatient hospice unit (anticipated in late 2016)

RN, nurse practitioner and physician staff in end-of-life care and communication
as well as quality metrics.

capabilities for accountable care and population health management.
One of 14 appointed members of committee created by Health Care Value Council to oversee implementation of the Post-acute Care Strategic Plan. Goal of
Initiative is to align efforts across the system to provide patients and families
with 'the right care, in the right place, at the right time'.
